Please feel free to print, fill out and bring these forms with you to sign-ups.

players MUST bring a Copy of their birth certificates with them the
day they are signing up.
(Do not bring originals as we have to keep
the copy for our records)

FOOTBALL PHYSICAL FORM:
All players trying out for football are required to have a physical examination before they can start conditioning and practice. Players not having a physical
will not be allowed to participate in the program.
Parents must turn in a Certificate of Physical Examination for Recreation
Activities Form and Parent Permission Form found by clicking the link below along with their child's registration form.
Do NOT turn in the actual physical from the doctor.
The doctor needs only to sign the Physical Examination for
Recreation Activities form.
